The Post holder will support the PCN Manager, Clinical Director and wider PCN team including the PCNs member practices, and multidisciplinary team (MDT) within the PCN with project work as well as general core support to the PCN with a wide range of administration and associated tasks across the network. They will assist coordination of all key activity including access to services, advice, information, and ensuring health and care planning is timely, efficient, and patient-centred.
Main duties of the job
Responsibilities include but are not limited to the following:
To work closely with the PCN Manager in supporting the Clinical Director, PCN practices and team in delivering the PCN Contract DES Specifications.
Undertaking a wide range of administrative duties and providing administrative support to the Primary Care Network and team, including arranging monthly PCN and MDT meetings, minute taking and recording.
Assisting the PCN Manager with their relationships and engagement with the PCN practices and other stakeholders across the locality to ensure a collaborative approach to service development and delivery.
Build and maintain PCN clinics on EMIS along with booking and cancelling patient appointments.
Utilising population health intelligence to proactively identify and work with cohorts of patients across the PCN e.g. flu clinics, COVID booster.
Supporting all member practices with data quality issues relating to PCN contracts, enhanced services and IIF/QOF.
Management of the PCN Website and support PCN digital work streams.
Working with the PCN Manager and DTL to develop and operationalise new PCN led projects and technologies as they immerge.
To support the PCN Manager and Practice Managers with Additional Roles Team alignment to the PCN practices e.g. HR issues, induction programmes, recording of mandatory and role specific training.
This role will require the post holder to be based at St Georges Wellness Hub, have the ability to visit PCN practices if and when required.

TheCare Coordinator responsibilities include but are not limited to thefollowing:
To work with the GPs and otherprimary care professionals within the Primary Care Network (PCN) to identifyand manage a caseload of patients who would benefit from support through carecoordination
To work closely and in partnership with the Social Prescribing Link Worker andHealth and Well-being Coaches to assist people to access self-managementeducation courses, peer support, health coaching and other interventions thatsupport them in their health and wellbeing, and increase their levels ofknowledge, skills and confidence in managing their health.
Work with patients, theirfamilies and carers to improve their understanding of the patients conditionand support them to develop and review personalized care and support plans tomanage their needs and to achieve better healthcare outcomes.
Identity unpaid carers and helpthem access services to support them and place them on the carers register
To support patients to utilisedecision aids, help create single personalised care and support plans, in linewith best practice.
To support the PCN in the delivery of the DES specifications, such astackling health inequalities through targeted work with specific groupsidentified through population health management
To help people to manage theirneeds through answering queries, making, and managing appointments andensuring that people have good quality written or verbal information to helpthem make choices about their care

* Must have some experience of working in general practice.
