We have an exciting secondment opportunity for an enthusiastic and self-motivated member of the care team to work as a Youth and Transition Worker to support the young people with life limiting conditions at Helen & Douglas House. This is a part time 22.5 hours per week 1-year fixed term maternity cover secondment and the salary ranges between £32,656 - £39,303 per year pro rata dependent on experience.
As part of this exciting role, you will support our young people with their lives, enabling them to be the best they can be and to achieve the best out of their life. You will achieve this by creating supportive, positive, and trusted relationships. This will enable our young people to find out what is important to them, and what they would like to achieve. You will work as part of a team supporting the delivery of the hospices Community and Family Support Services team and will be responsible for managing a caseload of young people aged 13-19 across various locations within the 6 counties of the catchment area.
We are looking for someone with knowledge in youth and transition work to support the young people and their families, offering information and advice, signposting, and referring where appropriate, and undertaking youth work activities in various settings.
Main Duties of the Job
1. To assess the needs of young people and plan and deliver programmes related to independence, practical and social skills, and relationships.
2. To link in with locality youth workers where available to see what facilities may be available externally for our patient cohort.
3. To run activities based on the abilities and needs of the patients, including day activities, outdoor education, and an introduction to sporting activities where available.
4. With volunteers, to befriend and support young people in different settings, including outreach work.
5. To work with families and young people in assessing individual transition needs.
6. To have a lead responsibility for the continual development of an appropriate transition pathway for young people visiting Helen & Douglas House; developing links with external agencies.
7. To work with families in identifying and researching any onward care services/provision from which the child and family may benefit.
8. To work in partnership with professionals from other organisations that support young people such as social care, health, and local authorities in supporting them through transition into adult services.

Person Specification
Qualifications Essential
* Recognised qualification in youth work or extensive experience. Evidence of continuous professional development.
Experience Essential
* Experience of working with children and young people with complex health needs. Experience of supporting children and young people through transition into adult services. Experience of working within a multidisciplinary team. Ability to prioritise and cope with competing pressures. Understanding of the complexities around safeguarding and how to escalate concerns. Understand the need for confidentiality.
Desirable
* Palliative care experience. Experience of working with other agencies and organisations.
Skills Essential
* Ability to plan and implement appropriate activities and programmes for the patient cohort. Evidence of well-developed communication skills both written and verbal. IT literacy. Ability to manage difficult situations.
