To assess, investigate, diagnose, plan and implement care for patients presenting with diagnosed and undiagnosed conditions and address continuing medical needs To work collaboratively with the general practice team to meet patient needs and cover duty days with other nurse practitioners as part of a triaging team. The practitioner will need to prioritise and triage the needs of patients accordingly making any necessary referrals for investigations in the appropriate manner. The practitioner will take part in the management of some patients with long-term conditions. Part of your responsibilities may include visiting patients in their home to assess their medical condition, at the request of the lead clinician. Diagnosing and treating patients presenting with minor illnesses Ensure clinical practice is safe and effective, and remains within boundaries of competence. Advise patients on health care and minor ailments with referral to other members of the primary and secondary healthcare team as necessary Liaison There is a need to work closely with all members of the Practice including the reception and administrative staff, as well as the medical and nursing team. There is also the need to establish and maintain good liaison with other surgeries and other agencies, including the Community Nursing Team, and Secondary Care. Help patients understand the role of a Nurse Practitioner within the wider practice team.
