We have an exciting opportunity for a Release and Transfer Nurse to join our Primary Care team at HMP Pentonville, a category B/C Prison located in North London.
HMP Pentonville has a capacity for 1,100 and primarily holds remand and convicted male prisoners. A significant portion of its population is on long-term remand. There is free parking on site.
Hours
Monday - Friday - 37.5 hours per week.
Salary
This position is full time, you'll receive an annual salary up to £50,000 per annum FTE - depending on experience.

What you'll be doing
As a Release and Transfer Nurse, your responsibilities will vary. You will:
* Use your expert clinical skills and demonstrate judgement in assessing and managing wide-ranging and complex patient problems.
* Manage the assessment of health needs, planning, delivery and evaluation of care for the prison population, including clinical risk assessment.
* Provide care and planning for adult male prisoners from the point of reception until the day of release or transfer to another establishment.
* Work collaboratively with the patient and any other agencies to support a safe transition back into the community or other establishment.
* Conduct a holistic patient assessment to include Clinical, Psychological, Substance Abuse, Social, Educational, Training and Employment, and Spiritual/Chaplaincy.

* Registered Nurse with current NMC registration.
* Must have prison experience - Essential.
