Youll work closely with a range of ARRS professionals including Clinical Pharmacists, First Contact Physiotherapists, Social Prescribers, and Health and Wellbeing Coaches alongside our practice staff to improve outcomes for our local population. Key responsibilities: Provide holistic, person-centred care to a defined patient list, with protected time to focus on continuity and proactive management. Contribute to the delivery of the GMS core contract specifications including QOF, Enhanced Health in Care Homes, Anticipatory care, and Personalised care. Work as part of a multi-disciplinary team within the practice and PCN, supporting integrated working with Acute, community, social care, and voluntary sector partners. Participate in quality improvement activities, data audits, and clinical supervision where appropriate. Engage in regular meetings and neighbourhood-level planning for population health management.
