Job Title: Community Care Officer
Location: Haringey
Contract: 22 Umbrella P/H
We are looking for a Community Care Officer for a role in Haringey to work within the adults service.
Key Responsibilities:
1. To deal with telephone, written, and emailed enquiries and respond appropriately to customer enquiries, complaints, requests for information, and service requests.
2. To assist customers in their understanding and completion of relevant documents, forms, applications, and registrations, over the telephone, including where appropriate addressing their special needs (disability, translation, interpretation requirements, etc).
3. Refer to other agencies.
4. Order equipment for clients (following training).
5. Amend referrals.
6. Screen referrals to gather further information.
7. To process referrals and apply agreed eligibility criteria. This will involve collecting extra information from the referrer, service user, or carer where appropriate.
8. To accurately record detailed referrals, specifying client's needs.
9. To work within agreed procedures, particularly those in relation to alleged safeguarding adults.
