This is an exciting opportunity to join the Discharge to Assess (D2A) Team in Moray as a Generic Band 3 Occupational Therapy & Physiotherapy Support Worker.
D2A is a therapy-led team working over 7 days a week supporting the early discharge of patients to home from hospitals across Moray. Patients are assessed in their home environment by the D2A team comprising Occupational Therapy, Physiotherapy, Advanced Nurse Practitioner and Generic Support Workers who provide support on discharge with a rehabilitation focus to maximise safety and independence in activities of daily living. Brief, focused, goal-orientated assessment aims to optimise patients’ independence, self-management and the promotion of health and wellbeing.
The post holder will be able to demonstrate hospital or community based skills as a support worker and be willing to undertake further training. You will be clinically supervised by a qualified Occupational Therapist or Physiotherapist and be able to carry out Occupational Therapy and Physiotherapy programmes as prescribed by qualified staff.
The Moray Occupational Therapy Team is a well-established team with robust supervision, mentoring, quality review and audit frameworks and we strive to provide an excellent service to our patients. We also have close working relationships with our AHP and multidisciplinary colleagues in Moray and across NHS Grampian and national links.
