Your knowledge spans relevant legislations like the Care Act 2014, Mental Capacity Act 2005, and Human Rights Act 1998. You have a proven track record in multi-disciplinary collaboration, conducting needs assessments, strategy meetings, and implementing complex care plans. You are skilled in IT, communication, and managing competing demands, with the ability to engage individuals in complex situations effectively. Awareness of current issues facing Social Workers complements your capability to understand confidentiality and data protection requirements. What the Job Involves Your role involves supporting the delivery of health and social care functions for the Wirral community, with a focus on Personalisation and Inclusion Pathways. You are expected to provide social work input and support adults, carers, and families, promoting self-determination and autonomy. This includes performing needs-led assessments, planning and reviewing care with a multi-disciplinary team approach, ensuring your practice aligns with professional standards. Your duties extend to organizing multi-disciplinary meetings, undertaking Adult Safeguarding inquiries, and upholding the principles of well-being and rights, including prevention of hospital admissions. You communicate complex care and risks effectively, submitting funding applications with healthcare colleagues, and ensuring equitable access to social care resources. Participation in community forums, compliance with supervisory requirements, and engagement in the development of educational materials for staff and partners are crucial, alongside contributing towards preventing and responding to abuse or neglect cases.
