Job summary CCN and Practice Development Nurse, Children's Community Nursing Team. This is an exciting opportunity to be part of an expanding service to the varied demographic of Croydon to optimise the health and well being of children and young people. The Children's Community Nursing Team provide holistic care to children and young people with acute, highly complex and unstable long-term conditions in partnership with the multiple disciplinary team, statutory and voluntary organisations. We are fortunate to have Specialist Nurses that work in partnership with our team inclusive of a Neurodisability CNS, Transition CNS, Oncology CNS and Epilepsy CNS. In this combined role the successful candidate will work both as The Practice Educator and hold a small caseload as a CCN. By having this combined role and working clinically this will ensure they are able to maintain their own clinical practice as a CCN, supporting their own and colleagues professional development. They will be a part of the senior leadership team developing community services for children and young people and ensuring care is evidence based and inline with Local and National Guidelines. The post holder will also have the opportunity to work as part of the wider Paediatric Nursing Practice Development Team and will receive clinical supervision with these colleagues. There will be the opportunity to undertake postgraduate teaching qualification at masters level and Professional Nurse Advocate training. Main duties of the job To act as a role model and demonstrate expertise in delivering evidence based practice. To support Matrons in mentoring clinicalpractice and meeting professional development needs of all staff to ensure that patient care standards and their experience are exemplary and any poor practice is identified and action plans implemented. The post holder will work alongside the senior management team to implement robust governance in relation to staff training and development, NICE guidance, the development of policies and procedures relating to children's community nursing and staff training and support following clinical incidents and RADAR reviews. The post holder will work in accordance with the Department of Health, Nursing and Midwifery Council and trust policies relating to infection control, safeguarding children/adults, standards of record keeping, medicines management, learning disabilities, privacy and dignity and other policies as appropriate to the designated areas.
