Main duties of the job

This Advanced Nurse Practitioner role will involveworking alongside a multidisciplinary team, including GPs, Nurse Practitioners,Clinical Pharmacists and Nurses to provide care and treatment to patients witheither long term conditions or undiagnosed undifferentiated diagnoses.
The successful Advanced Nurse Practitioner will work asan autonomous practitioner, diagnosing, managing, prescribing and dischargingpatients with illness or injury across all age groups including young children.This may be by face-to-face consultation, telephone or video consultation.

The successful post-holder will manage a caseload anddeal with a wide range of health needs in a primary care setting, ensuring thehighest standards of care for all registered and temporary patients.
In accordance with the practice timetable, as agreed, thepost-holder will make themselves available to undertake a variety of duties,including surgery consultations, telephone consultations and queries, visitingpatients at home, checking and signing repeat prescriptions and dealing withqueries, paperwork and correspondence in a timely fashion.

10-minute consultations
Share of home visits.
Able to manage acute presentations
Able to manage chronic conditions
Able to order appropriate investigations(bloods/radiology).
Able to refer appropriately to secondary care or organiseemergency admissions.
Share of admin: clinical letters which requireactioning/filing.
Able to file pathology results
Able to see all age groups
No restrictions on prescribing.
Able to see pregnant women in regards non pregnancyrelated issues.
Able to deal with mental health presentations
Person Specification
Qualifications
* Registered Nurse with Nursing and Midwifery Council
* Masters degree OR Post graduate diploma or degree for Advanced Practice Qualification OR RCN accreditation Qualified Independent Nurse Prescriber
* Meets the standards for registered ANP working at advanced level Minor illness qualification
* Experience in Primary Care
