Job Summary: The Advanced Clinical Practitioner (ACP) is a highly skilled healthcare professional who provides expert clinical care and leadership within the general practice. The ACP will work autonomously and collaboratively with the healthcare team to deliver high-quality, patient-centred care, focusing on assessment, diagnosis, treatment, and management of patients with acute and chronic conditions. Key Responsibilities: Clinical Practice: Conduct comprehensive patient assessments, including history taking, physical examinations, and diagnostic investigations. Diagnose and manage a wide range of acute and chronic medical conditions. Prescribe medications and treatments in accordance with practice guidelines and legal requirements. Provide expert clinical advice and support to patients, families, and carers. Leadership and Management: Lead and support clinical initiatives to improve patient care and practice performance. Mentor and supervise junior staff, including nurses and healthcare assistants. Participate in clinical governance activities, including audits, risk management, and quality improvement projects. Education and Training: Provide education and training to healthcare staff, promoting continuous professional development. Stay updated with the latest clinical guidelines, research, and best practices. Participate in personal and professional development activities to maintain and enhance clinical skills. Collaboration and Communication: Work collaboratively with GPs, nurses, and other healthcare professionals to ensure integrated and coordinated care. Communicate effectively with patients, families, and the healthcare team to ensure clear understanding and continuity of care. Participate in multidisciplinary team meetings and case discussions. Patient Safety and Quality of Care: Promote a culture of safety and quality within the practice. Ensure that care is delivered in accordance with clinical guidelines and standards.
