We are seeking to recruit a Health Care Practitioner specialising in Older People to join our friendly team in the lovely town of Sherborne.
You will be joining our existing Frailty Nurse providing a proactive role within the Sherborne PCN MDT, to identify and support the holistic assessment of our elderly patients. Working collaboratively to provide the best care for frail older adults, both living independently and in Care Homes.
With a caring and compassionate outlook, you will responsible for delivering effective comprehensive geriatric assessment. Compiling Care Plans, and supporting patients discharged from secondary care.

Although based at The Grove Medical Centre, your rolewill involve meeting patients in their own homes, care homes, and working fromour other two practices: The Apples Medical Centre and Yetminster HealthCentre.
This is a full-time position, currently Monday to Friday (some flexibility is possible).
A current driving licence and vehicle will be required.
Main duties of the job
Working closely with GPs, nursing teams and other staff of the MDT across three practices, and in the community. Supporting vulnerable older adults and their families, acting as the link between the MDT, patients, and their relatives / carers.
Utilising excellent communication skills to manage a caseload of patients, promote collaborative working, and adapt to an ever-changingenvironment.
The post holder will use specialist knowledge and skills to support the delivery of specialist nursing care, to help create an educative and supportive framework for geriatric care.

Sherborne Primary Care Network consists of three local practices, The Grove Medical Centre, The Apples Medical Centre and Yetminster Health Centre, all working together for the benefit of our local community. The PCN is rapidly expanding to allow us to provide a variety of specialist services to our patients, this includes a Pharmacy Team, Social Prescribers, Frailty Clinicians and Mental Health Clinicians.

Job Summary
To continue to develop the frailty service for the PCN working closely with the PCN team and Practice staff.
Participate in the care and development of anticipatory and current care planning for identified practice patients from all three practices in the PCN, to include treatment, preventative care, screening, future planning, and patient education in the practice and also in the patients home.
To act as a focal point of communication between patients, doctors, nurses, and other members of the primary health care team within the practices and community teams at Dorset Healthcare.

* Qualifications/Certificates or equivalent, in wound management, adult vaccinations and immunisations, phlebotomy
* A demonstrable commitment to professional development
* Qualifications/Certificates in compression bandaging and management of one or more long term conditions.
Knowledge and skills
* Competent in basic nursing duties
* Knowledge of wound management, adult vaccinations and immunisations and phlebotomy.
