We are looking for a forward-thinking individual to join our CAMHS/ CALDS Children's Continuing Care Team to cover a period of maternity leave. The post holder will ensure the provision of seamless, integrated services or package of care for individuals who meet children's continuing care criteria. These are services out with core and specialist provision. The successful candidate will have a key role in the implementation of the National Framework for Children's Continuing Care within the mental health and/or learning disabilities domains and will provide a case management function to a caseload of patients/service users receiving Children's Continuing Care, ensuring regular monitoring and review of care delivery is that of a good standard.
This post is fixed term/secondment for 12 months due to maternity cover.

Main duties of the job
1. To provide a case management role to a caseload of patients/service users.
2. Contribution to the development and implementation of an appropriate package of care.
3. Clinical compliance monitoring, implementation of a robust review programme, and provision of ongoing guidance and support to the nominated Care Coordinator.
4. To support the decision-making process by acting as chair in multi-disciplinary team meetings to ensure that process is robustly adhered to.
5. To ensure that commissioning decisions are lawful and reflect National priorities and agreed national and local service strategies.
6. To provide direct support to Care Coordinators in the development of new referrals for Children's Continuing Care applications including the sourcing of appropriate providers and packages of care for patients/service users who meet Children's Continuing Care criteria.
7. To quality assure Children's Continuing Care applications for patients/service users on colleagues' caseloads.
8. To attend the team Quality Assurance/Peer Review Panel to present cases and be responsible for the follow-up of any action as necessary.
9. To report ongoing/developing concerns relating to provider performance, updates on Corrective and Development Action Plans etc as necessary.
10. To undertake reviews for patients/service users on the caseload to ensure contractual arrangements are being adhered to in relation to the delivery of clinical care.
