This post is an exciting opportunity for someone who is keen to take the D2A service in Mid Notts to a new level. We are in the process of improving our D2A service as ensuring that we actively support the patient flow from hospital in the community, ensuring that patients are being discharged in a timely manner from hospital improving patients' health outcomes. You will be part of the larger community therapy team whilst focusing on our D2A service.
In your role you will work closely with Occupational Therapists, Assistant practitioners, support staff, clinical assessors (UCR team), district nurses, specialist nurses and social workers to provide the best possible care to service users in their own homes.
Main duties of the job
An exciting opportunity has arisen to join our developing integrated community services providing clinical triage, rehabilitation, falls management and support timely discharges from our acute Trust partners.
You will work autonomously as well as part of the multi-disciplinary team, liaising closely with health and social care colleagues to coordinate services, promote recovery from illness and enable health, wellbeing and self-care. You will be visiting patients within their own homes using a goal-oriented focus, to maximise patient's independence, safety and quality of life.
The post will involve cross site working and community visits. A full UK driving licence and vehicle for business use is essential for this post however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.

You will manage your own caseload in the community. As part of the team you will work closely with the rest of the MDT to provide the best possible outcomes for our service users.
You will have a degree/diploma in Physiotherapy and be HCPC registered. You should possess excellent interpersonal and time management skills, be a good communicator and able to prioritise and work well as a member of a team. An induction period and supernumerary support will be given. You will have significant experience of working in the NHS in the UK as a Band 5 Physiotherapist and being part of NHS healthcare services.
