We are looking for an experienced and compassionate Clinical Care Manager to support the clinical side of our domiciliary care business. The ideal candidate will have a strong clinical background and a passion for providing high-quality care to individuals in their own homes. As a Clinical Care Manager, you will play a pivotal role in supporting the Registered Manager to ensure that our customers receive the highest standard of care and support.
Key Responsibilities:
* Clinical Oversight: Provide clinical support and guidance to care staff, ensuring the delivery of safe, effective, and compassionate care.
* Care Planning: Develop, implement, and monitor personalised care plans for customers, ensuring they meet individual needs and comply with regulatory standards.
* Quality: Conduct regular audits and reviews of our care services to ensure quality standards are being met. Address any concerns or issues promptly and effectively.
* Compliance: Ensure that all care practices comply with relevant health and safety regulations, industry standards, and company policies. Maintain accurate and up-to-date records for all customers, ensuring proper documentation and reporting in accordance with company procedures and regulatory requirements.
* Family Support: Serve as a point of contact for customers and their families, addressing concerns and providing accurate information on care and treatment.
* Collaboration: Work closely with other healthcare professionals such as doctors, physiotherapists, district nurses and social workers and have the ability to coordinate care and ensure the best possible outcomes for clients.
Required Qualifications/Experience:
* Proven experience in a clinical or care management role, preferably in a domiciliary care setting.
* Strong understanding of healthcare regulations and compliance standards.
* Excellent communication, organisational, and leadership skills.
* Ability to work independently and as part of a team.
* A compassionate and patient-centred approach to care.
* Ability to work independently and as part of a multidisciplinary team
* Experience with care planning software and electronic health records.
* Knowledge of safeguarding protocols and procedures.
