Job Title: Community Link Worker
Employer: Voluntary Action Angus, 5-7 The Cross, Forfar DD8 1BX
Hours of work: 35 hours per week
Salary Scale: £25,000
Role Overview: To deliver Community Link Working in localities in Angus, taking additional responsibility for connecting with wider VAA Locality workers, third sector and strategic partner opportunities, and connecting GP practices with community-based opportunities which help improve wellbeing.
Key Responsibilities:
1. Support personal options and choices for patients to help improve wellbeing.
2. Refer patients to comprehensive social care services.
3. Target groups and communities most in need to help them live healthier, happier lives.
4. Work with a wide range of people including GP’s, statutory partners, and voluntary organisations.
5. Assist patients with long-term conditions, loneliness, poverty, inequalities, drug and alcohol issues, and complex social needs.

Qualifications

Experience
1. Experience in community development, adult health and social care, or public health.
2. Experience supporting people and their families in a related role.
3. Experience in supporting mental health.
4. Experience working with the VCSE sector.
5. Experience in data collection and measuring service impact.
6. Experience in partnership and collaborative working.
7. Knowledge of personalised care approaches.
8. Understanding of the wider determinants of health.
9. Knowledge of community development approaches.
10. Understanding of equality, diversity, and inclusion.
11. Knowledge of IT systems.
12. Knowledge of local VCSE and community services.
13. Knowledge of HHS workings including Primary Care.
Other
1. Willingness to work flexible hours.
2. Access to own transport and ability to travel regularly.
Operational Development and Duties
1. Devise and implement effective pathways for patients addressing holistic needs.
2. Complete ‘soft handover’ processes with patients.
3. Develop and promote Community Link Working.
4. Process and update monitoring information for clients.
5. Produce monthly and quarterly reports.
6. Ensure end-of-programme evaluations and reports are produced.
7. Participate in training and development as determined by the line manager.
8. Market and represent the CLW Project widely.
Other Duties
1. Ensure adherence to relevant legislation and VAA and GP practices.
2. Participate in ongoing supervision and appraisals.
3. Attend Health Centres meetings and participate in MDT activity.
4. Create and maintain good working relations with HSCI members and clients.
5. Operate Health and Safety procedures.
6. Carry out any other reasonable duties required by the VAA Manager.
7. Work as a team member with senior workers and partner agencies.
