Full job description
Main duties of the job include:
1. Use assessment tools, VDT screening tool, and OT formulated tools for the community team.
2. Conduct clinical assessments and occupational therapy screenings, and write reports.
3. Track changes in assessment tools/outcome ratings.
4. Liaise with staff and hostels to run groups and schedule appointments.
5. Run therapeutic groups in the community for discharged service users; providing transport support where necessary.
6. Conduct 1:1 sessions with service users who have specific goals or needs that require support, such as:
1. Transport training
2. Joining college
3. Accessing community groups and pursuing existing interests/hobbies
4. Attending leisure centres
5. Finding structure and meaningful occupations in the community during the week
6. Volunteering and vocational goals
7. Support in building healthy living skills
8. Budgeting skills
