About The Role
About you
We are looking for someone who can:
1. is passionate about supporting people to maintain their independence, improve their sense of well-being, and help them to live well with dementia.
2. has professional or personal experience of dementia and the challenges people affected by it may face.
3. is comfortable working in the community – meeting people in their homes to deliver dementia support.
4. is able to manage difficult conversations and adopt a non-judgemental approach.
5. can network effectively and collaborate with other professionals to achieve positive outcomes for people.
6. is capable of managing a caseload of service users effectively and independently, ensuring face to face reviews and telephone contacts happen within expected timescales.
7. has strong written communication skills and can write case notes with clarity, accuracy and timeliness. Previous experience of writing case notes is not required.
8. has the ability to travel across the local area independently as often as required

What you’ll focus on:
9. Managing an existing caseload of service users as well as new referrals to deliver ongoing post-diagnostic support.
10. Working collaboratively with NHS clinicians to ensure the right support is offered to people at the right time.
11. Writing and updating of ‘Wellbeing Plans’ that capture a person’s history, current support needs, and future wishes.
12. Supporting our peer-support groups, delivering carers education courses and engaging in awareness raising events in the community.
13. Identifying service user support needs and referring onwards to appropriate organisations (e.g. social services, other charity organisations).
