To carry out the duties as outlined and be proactive in continuously improving service delivery. To work in a professional manner with a client focus, ensuring that respect and courtesy is shown to patients, colleagues, other service providers and all those in contact with the organisation. Our staff are expected to be flexible to work at any/all our PCN sites and in patients' place of residence when clinically necessary. Following the release of ARRS GP information and funding, we are looking for newly qualified (within 2 years and not having had a substantive post), enthusiastic, forward-thinking GPs to join Middleton & Hunslet PCN on a fixed-term salaried contract until 31 March 2026.
We will consider applications for between 2 and 8 sessions with a start date as soon as possible. You will be required to work across 4 practices (4 sites) supporting our practices through the winter period. We are a highly supportive PCN, and the practices all work closely within the PCN, understanding the importance of a good work/life balance with an open-door ethos.
Responsibilities:
- Undertake duties governed by the PMS & GMS contract and Objectives of Direct and Local Enhanced Services including:
1. Face to face and telephone consultations
2. Patient home visits and nursing home visits
3. Checking and signing repeat prescriptions
4. Administration - dealing with queries, patient test results, referral letters NHS/private, tasks, emails, and other paperwork and correspondence in a timely manner
5. Cover all the clinical management of the patients in surgery as appropriate including initiating investigations, reviewing results, making referrals to secondary care or to other providers as appropriate
6. Develop care and treatment plans in consultation with patients and in line with current Practice disease management protocols as well as wider guidance such as that from NICE
7. Provide safe, evidence-based, cost-effective, individualised patient care within the surgery, patients' own home or other environment where patient care is carried out
8. Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly referring patients to other services/agencies in a timely manner, utilising Practice, PCN and local guidelines
9. Recognise the need for and be able to perform and interpret investigatory procedures, including laboratory and radiology
10. Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care
11. Ensure appropriate follow up of patients
12. Support patients to adopt health promotion strategies that promote healthy lifestyles and apply principles of self-care
13. Support the delivery of anticipatory care plans
14. Recognise and work within your own competence and in accordance with professional codes of conduct, maintaining accurate and contemporaneous health records
15. Independently prescribe for patients within your scope of practice, in accordance with locally agreed or national guidelines
16. Compile and issue computer-generated acute and repeat prescriptions
17. Review medications following the appropriate policies, NICE/local clinical guidelines and local care pathways
18. Maintain an awareness of developments in clinical practice
19. Be aware of your duties and responsibilities regarding current legislation and adhere to our policies and procedures on Safeguarding Children and Safeguarding Adults
20. Contribute to the achievement of the highest possible quality standards such as those detailed by their regulatory body and the CQC
21. Monitor the safety and effectiveness of own clinical practice through quality assurance strategies such as the use of audit, mentor feedback, case review and peer review
22. Effectively manage own time, workload and resources
23. Work in partnership with other clinical teams, collaborating on improving the quality of health care responding to local and national policies and initiatives as appropriate
Communication:
- Excellent communication skills (written and oral), when dealing with patients and other team members.
- The ability to make clear decisions with confidence and communicate these effectively.
- Demonstrate sensitive communication styles to ensure patients and carers are fully informed and consent to treatment.
Managing Risk:
- Manage and assess risk within the areas of responsibility, ensuring adequate measures are in place to protect staff and patients.
- Apply infection control measures within the practice according to local and national guidelines and in accordance with the practice policy.
