Responsible for ensuring that our service delivers itsoptimumperformance, you will work to achieve a seamlessservice. Driven to improve patient experience, you will be part of the triage team assessing referralsin a timely mannerand signposting to other teams and services delivering care needs.Your energy and drive will be rewarded with the support of a dedicated team that strives to create a warm andprogressive environmentin providing outstanding care for our patients.
• Provide clinical support,assistance, and professionalsexpertiseto all callers to the Community Nursing Triage Telephone Support.This will range from signposting the referral to theappropriate service, making a direct referral, or offering advice.
• Work in conjunction with administrative support staff to provide high quality, clinical triage using knowledge, skills, critical thinking, and professional judgement
• Control and manage calls effectively by using skilled questioning, active listening and excellent verbal communication skills in a calm, professional manner.
• Possessthe ability to prioritise tasks,cases,and situations appropriately to ensure effective delivery of services during periods of high demand.
• Possess relevant clinical skills to be able to visit urgent visits in the patient’s home, such as catheter care and end of life care.
