The Adult Mental Health Social Worker will work as part of a multidisciplinary team to support individuals with mental health needs, ensuring they receive appropriate care and intervention. You will provide assessments, care planning, and ongoing support to people with complex mental health needs, helping them to lead as independent and fulfilling lives as possible.
Key Responsibilities:
* Conduct comprehensive assessments of individuals with mental health conditions, evaluating their physical, emotional, and social needs.
* Develop, implement, and review care plans in collaboration with service users and their families.
* Support individuals to access relevant mental health services, resources, and interventions.
* Provide case management and advocacy for individuals to ensure they are receiving the necessary services and support.
* Liaise with healthcare providers, housing, and community agencies to coordinate care and services.
* Participate in team meetings, supervision, and continuing professional development activities.
* Maintain accurate and timely records of assessments, care plans, and interactions with service users in line with safeguarding protocols and confidentiality guidelines.
* Provide expert social work support in crisis situations, ensuring effective risk management and decision-making.
* Promote social inclusion, offering guidance on housing, employment, and social integration.
* Ensure that all interventions are culturally sensitive and tailored to the individual's unique needs.
Essential Criteria:
* Qualification: Recognised Social Work qualification (e.g., BA or MA in Social Work).
* SWE Registration
* Experience: Experience working with adults with mental health issues, ideally in a community or clinical setting.
* Knowledge: Knowledge of relevant mental health legislation including the Mental Capacity Act 2005 and safeguarding procedures.
* Skills: Excellent communication, assessment, and case management skills.
* Personal qualities: Compassionate, resilient, and committed to working with vulnerable individuals. Ability to work independently and as part of a multidisciplinary team.
* Car Driver: A full UK driving license and access to a vehicle is required for this role.
