Then this is the job for you You will lead, motivate, and empower a team of qualified and unqualified team members, to safely discharge Hertfordshire citizens from hospital, on the right Discharge to assess pathway. Our Barnet General Hospital Team is an integral part of the Herts Valley Integrated Discharge Service, operating across the South & West Hertfordshire & East and North Hertfordshire. Working in partnership with key stakeholders, the team will work with Hertfordshire citizens and their carers to implement a safe and sustainable discharge from hospital. Implementing the Home first agenda, where possible, citizens will leave hospital with short-term services that support their recovery, meet their outcomes, and help them to live healthy, independent lives. To improve outcomes for service users, the Integrated Hospital Discharge Service supports a 7-day Hospital Discharge service. The post will be asked to be part of the service-wide rota to provide cover on consecutive Saturdays and Sundays once every eight weeks and will receive days off during the weeks before and after covering weekends. About the role As Team Manager, you will report to the Service Manager and be responsible for daily management and performance of the team, along with management, control and accountability of financial budgets. Overseeing referrals into the team from hospitals, ensuring that they are recorded on our database, and allocated to a worker upon receipt. The role requires you to demonstrate in practice, your knowledge and application of legislation and statutory guidance, including but not limited to the Care Act 2014, Mental Capacity Act 2005, Human Rights Act 1988 and the Hospital Discharge and community Support Guidance. You will lead by example, sharing good practice, and learning, particularly when we don't get things right through complaints, investigation, and response. Key activities in this role include; Work cross-organisationally as a leader of the Transfer of Care Hub. Shape services, through participation of projects, and workstreams. Support and develop staff to reach their potential. As well as your own. Be an integral part of a service that champions Human Rights, and advocacy. Make a difference to individuals and carers lives. recruit and support staff who share the Hertfordshire County Council's values and behaviours. Manage budgets, and shape resources. Lead and showcase your skills. About you Essential Qualified Social Worker, registered with Social Work England. However, we would invite applications from Occupational Therapists or Registered Nurses. Drive or have access to a vehicle for the purposes of work, as the role requires travel to other locations Including but not limited to Hospitals, individuals' homes, Council and NHS Offices, Care Homes within and outside of the Hertfordshire's Boundaries. You will need to have a valid and clean drivers' licence as a requirement of this post. Ability to write reports to court standard, case notes to the required professional standards defined within your professional registration standards and Hertfordshire County Council's policy on record keeping. Have knowledge of and be able to demonstrate application in practice of the Care Act 2014, Social Work Models, Mental Capacity Act 2005, Deprivation of Liberty Safeguards and Community, Human Rights Act 1988, and the Mental Health Act 1983. Be part of a weekend rota. 1 weekend in 8, on consecutive Saturdays & Sundays. Be part of the Bank Holiday management rota to ensure the service is covered 364 days a year.
