We are currently recruitingfor a dedicated and motivated Band 6 Senior Community Nurse tojoin our community nursing team covering the borough of Woking.
The Community Team visit patients in their own home as well as care homes. There are excellent opportunities for training and development of both your clinical and leadership skills. The team has experienced Clinical Leads who will ensure that you have the necessary support for your personal development. The community nursing service works over a 7-day week 8.00am to 8.00pm offering a variable shift pattern, and includes weekend and bank holiday working.
Individuals will be expected to deliver and maintain a high standard of clinical and professional Nursing Care and to assist in the delivery of all aspects of individualised patient care.

Job responsibilities
Responsible for the delivery of care and for managingservice provision in accordance with national and local priorities,organisation policies, procedures and guidelines and in accordance with theNursing and Midwifery Council.
Principal duties to include:
* Responsibilityfor a defined caseload of patients
* Is thenamed District Nurse for designated GP practices and attends all GSF and MDTmeetings
* Undertakesfull nursing assessments of patients, including those with multiple pathologyand complex health and social needs
* Responsiblefor agreeing the patients care plan, liaising with others and adapting thecare and treatment delivered according to need and patient choice
* Responsiblefor the monitoring of medication regimes, assessing effects and advisingpatients on the safe storage and disposal of drugs
* Responsiblefor the correct use of aids and equipment supplied for patients and carers andfor the security, care and maintenance of equipment ensuring standards ofinfection control and safety are maintained including equipment loaned topatients
* Recognisesown limitations in the provision of clinical care and urgency of patientsneeds, referring to other health care professionals accordingly and isaccountable for his/her own actions often without direct supervision
* Maintainsaccurate and legible patient records in line with professional andorganisational requirements this includes contemporary paper records andelectronic patient record system
* Accountablefor making autonomous clinical decisions in planning and prioritising patientcare for District Nurse caseload including referrals to other agencies and theadmission / discharge from the service
* Responsiblefor recognising the needs of the service, identifying risk and making rapidautonomous decisions, escalating to the Team Lead and Clinical Lead as required
* Triagingwithin the District Nurse Referral Centre and promoting and embeddingtechnology within the work place
* Operationalline manager for District Nursing team (Band 5 RNs / Band 4 APs / Band 3HCAs)
* Effectivelycommunicates with others involved in patient care at all times includingweekends / out of hours, through the use of mobile phone, emails and fax toenable a timely response to patients needs.
