West Berkshire Council is seeking a dedicated and motivated Care Practitioner to join our Adult Social Care Service, working within the Support Service for People with Disabilities. Role And Responsibilities As a Care Practitioner, you will be responsible for: Working collaboratively with the Support Outreach Workers to deliver person-centred care and support to individuals with disabilities. Conducting assessments of care needs and implementing appropriate care plans. Monitoring and reviewing care packages, ensuring that changes in needs are addressed. Providing advice, guidance, and support to service users and their families. Collaborating with external providers to ensure the effective delivery of care services. Maintaining accurate and up-to-date records of care provided. Working in partnership with other professionals to promote the overall well-being of service users. Skills And Attributes The ideal candidate will possess the following skills and attributes: A thorough understanding of the care needs of individuals with disabilities. Experience in delivering hands-on care and support. Excellent communication and interpersonal skills, with the ability to build positive relationships with service users, families, and colleagues. Strong assessment and care planning skills. Ability to work independently, as well as part of a multidisciplinary team. Good organisational and time management abilities. Knowledge of relevant legislation and best practices in care provision. A positive and compassionate approach to supporting individuals.
