Responsibilities
Conduct comprehensive assessments of clients' needs and challenges.
Develop and implement individualized care plans tailored to each client.
Advocate for clients by connecting them with essential resources and services.
Provide counseling and emotional support to clients and their families.
Collaborate with professionals across various disciplines to ensure holistic care.
Monitor and evaluate client progress, adjusting care plans as needed.
Maintain accurate and timely documentation of client interactions and progress reports.
Requirements
Master's degree in social work from an accredited institution.
Valid state license to practice as a social worker.
Minimum of 2 years of relevant social work experience.
Strong knowledge of social work principles, ethics, and best practices.
Excellent communication and interpersonal skills.
Ability to work independently and as part of a team.
Proficiency in case management software and Microsoft Office Suite.
Hours Per Week: 37.00
Start Time: 08:45
End Time: 17:15
Pay Per Hour: £23.74
