Main duties of the job
To support members of the OCT and ward staff to work effectively in discharge planning.
To ensure information, advice and services are available in a timely way to support discharge including ordering of equipment, onward referral when needed, keeping updated with any onward capacity issues and ensuring this is communicated clearly with ward staff.
To assist with the assessment of patients with the support of a qualified practitioner when needed.
To work as part of the ward multi-disciplinary team, leading pre-emptive discussions on discharge planning.
You will need to able to communicate effectively with people from all professions and levels within our organisation and beyond.
You will need to be professional and respectful as you will often be the link between the patient, their family, the ward and onward care providers.
About us
We are a team of therapists, nurses and non-registered clinicians who work across both sites within GHT to support the coordination of onward care planning for those patients who require some support when leaving hospital.
We are constantly adapting to the changing landscape within the community in relation to resources to support with discharge planning and as such, no two days will feel the same.
We will need you to be dynamic, engaged, happy to question things in a way that brings people with you, flexible in your thinking and organised in your daily tasks.
Job description
Job responsibilities
To independently case manage the discharge planning for a cohort of patients within a defined clinical area from admission to the acute hospital through to discharge.
In the absence of a Senior OCT clinician, to help coordinate provision of OCT cover to ward areas within their team.
To act as a resource providing advice and information to clinical teams to enable patients with onward care requirements to meet their expected date of discharge (EDD) safely and effectively.

1. Experience of working in an acute clinical environment
2. Previous experience in a relevant speciality
Knowledge/ Skills

3. Act as a role model and are able to lead by example to ensure the Trust's values and behaviours are reinforced throughout their area of practice
4. Able to effectively and appropriately escalate concerns to reduce risk and promote patient safety.
Desirable
5. Demonstrate an understanding of the roles of the different members of the MDT.
6. Able to contribute to the implementation, policies/protocols/guidelines relevant to area of practice.
