We support patients who are end of life across Bradford in the community to provide high quality care to patients at the end of life. Working closely with community nursing teams, Fast Track team and Community Palliative Nurse Specialist, you will support patients to prevent unnecessary admissions to hospital, and support them in their preferred place of care. In addition, you will work alongside other key members of the primary health care team, other health and social care professionals, and carers, to develop and implement personalised care plans for patients at the end of life.
