Job overview
As part of the Virtual Ward Team, the post holder will be responsible for the clinical assessment of the needs, development of programmes of care, and for the implementation and evaluation of these programmes for patients and their relatives.
To provide a high standard of clinical nursing within the ward/department to manage a group of patients.
The post holder will also be expected to coordinate the care of patients and staff allocation when rostered and assessed as competent to take on the roles and responsibilities of leadership and supervision.
To act as a role model and mentor to Nursing Associates, Healthcare Support Workers, Students, and Trainees.
To maintain a positive and professional image of the organisation and of the nursing team. Participate in MDT's and Board Rounds.
Main duties of the job
1. Communicate using a range of methods to ensure a caring, sensitive, and empathetic manner to patients and their relatives.
2. Escalate concerns in a patient's condition to senior members of the nursing team, medical staff, and initiate referrals to other members of the healthcare team.
3. Prioritise workload to manage the care of a group of patients.
4. Undertake digital assessments of the patients that will support remote monitoring and supporting the patients.
5. Ensure the delivery of holistic patient care recognising the individual needs of patients and their relatives.
6. Facilitate the admission and discharge of patients liaising with other agencies to ensure continuity of care between hospital and community services.
