We are looking to recruit a dedicated Care Coordinator to join our growing organisation.
You will be an integral part of a busy and successful team, ensuring the delivery of care to clients living in their own homes, and supporting them to live independent and fulfilling lives.
You will demonstrate outstanding communication and organisation skills as you will be liaising with both Community Care Workers and Customers, ensuring the highest level of customer service to deliver a safe and effective service that meets the requirements of both company expectations and those of the CQC.
As each customer is unique, every day will be different. Our priority is to support our customers live the lives that they choose, keeping them in control as far as they are able. You must have a good understanding of what people in need of home care and support require.
This role is predominantly weekdays 9 am-5pm with some on-call out-of-hours duties required.
It is essential you are happy to provide hands-on care to our customers if/when required.
MAIN RESPONSIBILITIES
1. Coordinating care packages and assigning workers to rotas
2. Accept, allocate and process new referrals for care and support promptly
3. Demonstrate decision-making skills and problem-solving
4. Ability to develop relations with colleagues across the business
5. You will be required to embrace the company's vision, values, and objectives.
ADDITIONAL RESPONSIBILITIES
1. Review and process changes to clients' care plans.
2. You must be able to build trusting relationships with customers, their families, staff, and other professionals such as district nurses, GPs, and occupational therapists.
3. Ensuring workers have the necessary skills, training, and competencies to undertake their shifts
4. Effectively communicate both verbally and in written form.
5. Arranging care reviews
6. Carrying out telephone monitoring of care packages
7. Recruiting new care workers
8. Ensuring care provision meets CQC guidelines
9. Adhering to company compliance standards
10. Excellent IT skills particularly in the use of Microsoft Office (Excel, Word, PowerPoint, Internet Explorer, and Outlook).
11. Take part in the out-of-hours emergency on-call rota
12. You must be able to work flexibly as a part of the Neighbourhood Care Team

KNOWLEDGE, SKILLS & EXPERIENCE
1. Experience in planning would be beneficial
2. Experience in healthcare and social care environment
3. Excellent organisational skills
4. Ability to work under pressure
5. Have good judgement and decision-making skills.
6. You will need to be self-motivated and enthusiastic with a passion to deliver exceptional care and support.
7. Full driving licence and access to a car essential
