Rate ofPay: 20.47 25.33 perhour
Responsibilities:
* To ensure our duties under the HomelessnessReduction Act are met delivering a proactive person centred outcomefocused and end to end case managementapproach.
* To undertake comprehensiveassessments that lead to focused and coproduced Personal HousingPlans (PHP) for each applicant.
* To assessapplicants under the prevention and relief duties issue S.184 finaldecision letters ensuring effective communication and management ofexpectations with customers.
* As appropriateundertake the assessment and completion of highquality riskmanagement plans for applicants utilising information from a rangeof stakeholders and the applicantthemselves.
* To minimise risk by identifyingreporting and following up any safeguarding concerns.
Qualifications &Experience:
* Significant knowledge of safeguarding best practice includingpolicy strategies legislation and case law and its implementationwithin a statutory homelessnessservice
* Significant experience of providingspecialist homelessness casework to single homeless applicants withmore complex needs such as applicants leaving institutionsincluding prisons and hospitals
* Significantexperience of working collaboratively to improve outcomes forsingle homeless applicants with more complex needs such asapplicants leavinginstitutions
* Ability and experienceto consistently deliver high levels of customer service to singlehomeless applicants with more complexneeds
* Ability and experience ofensuring all aspects of casework is carried out in accordance withlegislation strategies policies and initiatives set out at thenational regional and locallevel
* Ability and experience ofconsistently achieving performance objectives in the provision ofspecialist homelessnesscasework
* Ability to contribute to theidentification dissemination and embedding of existing and emergingbest practice within the team and across the wider Housing AdvisoryService
* Ability to effectivelycommunicate with a range of stakeholders including applicantsmembers partners and soon
* Significant knowledge of welfarehomelessness and housing policy strategies legislation and case lawand its implementation within a statutory homelessnessservice
