All members of staff should be prepared to take on additional duties or relinquish existing duties in order to maintain the efficient running of the organisation. This list may be amended, following consultation with the post-holder, to facilitate the development of the role, the Network and the individual. Main Duties: Provide face-to-face and telephone consultations for routine and urgent care. Manage patient care in surgery, including investigating and reviewing results, developing care plans, and making referrals to secondary care, the PCN team or other providers. Issue and sign repeat prescriptions, ensuring they align with national/local guidelines. Record clear, contemporaneous consultation notes using EMIS and S1 clinical systems. Ensure safe, evidence-based, and cost-effective care, in line with Practice clinical protocols and NICE guidance Independently prescribe medications within your scope of practice. Utilise other available clinical and support staff in delivering a holistic and patient centred care. Actively participate in MDT and care conference meetings, as required. Administer and handle patient-related queries, test results, referral letters, and other tasks in a timely manner. Conduct patient home visits if needed. We do have an Acute Visiting Service.
