The post holder will utilise skills of clinical expertise, leadership, education, and management to ensure a seamless service for patients, carers, and staff. The post holder will provide advice, support, and information and will work closely with the multidisciplinary team to ensure a co-ordinated quality service. You will be actively involved in ensuring a high-quality safe service for patient and staff leading on audit and research as appropriate.
Main duties of the job
To facilitate a safe and timely discharge of frail patients within ED using comprehensive geriatric assessment working alongside other members of the multidisciplinary team.
