This role is a relief role where the hours will be as and when required with the option of working both days and/or nights, on a zero hour contract. Write care plans and ensure these are followed and handed over to all staff, take an active part in hand over and report any concerns to the Management Team. De-escalate and manage stressful situations in a calm manner. Regularly fully utilise the Trust 'Client File' with designated clients to ensure that they are supported and empowered to achieve appropriate outcomes. To ensure that any medication within the Service is managed in line with Trust policy and procedures. Provide support to other Support Workers to assist them in the safe delivery of the service and to support client engagement.
