Job Description:
To provide an assessment, support planning and review service, underpinned by professional knowledge in order to ensure that:
* Vulnerable people are safeguarded and are supported in managing risk.
* Person centred outcomes and support requirements are identified.
* Customers are encouraged and enabled to live healthy and independent lives for as long as possible.
Responsibilities include:
* Working closely with colleagues in the health, early help, community and voluntary sectors to ensure person centred outcomes are met using available resources effectively.
* Working within organisational policy and legislative requirements.
* Undertaking professional assessments to assist customers in managing risk, identifying person centred outcomes, and conducting support planning and reviews.
* Delivering services in accordance with statutory responsibilities set out in NHS and social care legislation and the eligibility criteria for services as agreed by the Council.
* Undertaking best interest assessments in accordance with the Mental Capacity Act.
* Supporting carers in their role.
* Ensuring timely and person centred support plan reviews evidencing the need for continuing support.
* Providing information and advice to help people stay healthy, plan for their future needs, use services effectively, take responsibility for their own health and care, and maximize the impact of prevention and recovery services.
* Encouraging and supporting people to look after themselves, their families, and their communities, promoting choice and control for people, and the use of personal budgets and direct payments where possible.
* Undertaking straightforward financial assessments and providing welfare benefits advice, signposting to specialist services where appropriate.
