This role requires a clinically experienced, innovative, and proactive leader, working directly with, and taking responsibility for providing expert advice and support to, patients, carers, and other healthcare professionals, as part of the Community Hospital Pharmacy team.
The role requires working alongside their Advanced Clinical Pharmacist counterpart, using their clinical experience and leadership skills to positively and proactively lead, support, and develop the pharmacy team and the clinical pharmacy service. To ensure that by keeping patients at the center, we achieve the best medicines-related outcomes for the patients we serve across the Community Hospitals in KCHFT and beyond.
As part of an exciting new service development, we are exploring collaboration with a PCN to create joint pharmacist roles. The post holder will also support the development of these roles to provide high-quality, patient-focused services for the local community across boundaries.
