We have an exciting opportunity to appoint highly motivated Health Navigators to join our friendly and supportive Single Point of Access (SPA) team across Bristol, North Somerset, and South Gloucestershire on a permanent basis!
Our team is compiled of 100 Health Navigators & Clinicians working flexibly across 3 sites in Bristol, Clevedon, and Warmley. We offer hybrid working and flexible shifts. We are looking for Navigators who are available on during evenings and weekends.
SPA is the first point of contact for all referrals into Planned Nursing, Therapy and Urgent Community Response.
As Health Navigators we provide comprehensive administrative support to health and social care professionals, hospitals, GPs, service-users, their relatives and carers and other stakeholders in the wider health community. We provide first line support through phone calls, emails and other electronic referral.

We are looking for a team member who is hard working, flexible and able to prioritise their work, and who wants to be part of a highly motivated and enthusiastic team.
Main duties of the job
To act as the first and central point of contact for the service both by telephone, email and in person. To determine and / or act independently to enquires which may be of a confidential / volatile nature.
Have a good knowledge of various administrative processes and practices, some of which are non-routine activities such as answering queries, chasing progress and task-related problem solving acquired through experience and training
To have the ability to work without supervision when required to identify patients needs and the most appropriate pathway for people referred via SPA using a clear set of guidelines.
To liaise internally with colleagues within the organisation, and externally with partner organisations and outside agencies, service users, and other members of the public.
To maintain accurate and up to date patient records and data collated in the Sirona IT systems

Qualifications
Essential
1. Functional literacy and numeracy level 3/ GSCE grade A-C or equivalent including English Language and Mathematics
2. NVQ 3 Business and Administration or equivalent experience
3. Trained in a range of systems/IT applications relevant to the role eg EMIS/Clinical System and Microsoft Office
Desirable
4. Educated to AS/A Level Standard or equivalent
5. NVQ 4 Business and Administration or equivalent
Experience

6. Recognising complex patient needs and escalating appropriately
7. Able to handle sensitive and confidential issues and recognise the need for discretion and confidentiality
8. Proven experience of using an EPR (Electronic Patient Record) EMIS, Rio, System One
Desirable
9. Previous experience of Cloud based telephony systems
10. Proven ability to work from home and maintain effective workload prioritisation
11. Previous acute or community care experience
