An exciting opportunity has come up to join the Virtual Ward Team as a Healthcare Assistant as part of the wider Digital Health System within CFT.
This service is expanding to incorporate the ethos of admission avoidance and also providing hospital level care at home and requires dedicated and committed professionals to ensure its success and keep patients at the forefront of what we do.
This role will incorporate a hybrid way of working, caring for patients on the Virtual Ward completing online monitoring and also within their own homes completing a variety of tasks. You will enhance the role of the registered professional and work alongside an MDT to provide high levels of care to a variety of patients with acute medical needs.
* Please note that this job is a hybrid role working online and in the patients' own home.
Responsibilities include:
1. To assist in the delivery of clinical care, planned and assessed by the Registered HCPs.
2. To carry out care / other duties under indirect supervision of a Registered HCP.
3. Support care on a day-to-day basis for all patients in the clinical area in a manner sensitive to the individual clients' needs and wishes, always acknowledging their rights and beliefs.
4. Contribute to resource management within the clinical area.
