Job Summary
A fantastic opportunity has arisen for an Advanced Clinical Practitioner to join our team within Acute Internal Medicine, part of the Urgent Care CSU based at St James Hospital. This role is open to experienced and qualified practitioners.
The role will involve working across all Acute Medicine services, including a purpose-built Same Day Emergency Care (SDEC) unit, a Higher Observation Unit, and 2 busy acute medical units.
This is an exciting opportunity for a qualified and experienced ACP to apply their transferable skills while expanding their knowledge in the management of acute medical presentations. A short preceptorship period will be negotiated depending on individual needs. We pride ourselves on providing a supportive environment.

Main Duties of the Job
Clinical supervision, teaching, training, and ongoing development will be available as part of the Acute Medicine ACP Development Programme, which will include a dedicated programme for qualified ACPs. The post holder will be required to demonstrate continuous professional development across all 4 pillars of advanced practice.
The successful applicant must:
1. Have considerable, demonstrable experience within acute medicine as an autonomous practitioner.
2. Be clinically competent to manage adult presentations across the spectrum of illness and injury.
3. Hold an MSc qualification in Advanced Clinical Practice, including a non-medical prescribing qualification.
As a registered professional with a Master's in Advanced Clinical Practice, you must demonstrate significant clinical experience. You will need to be highly motivated and committed to continuing your professional development. Your role will involve the assessment, treatment, and management of a diverse, undifferentiated caseload of patients. You will also need excellent communication skills and the ability to work as part of the wider team, including collaborating with other specialties.

Job Responsibilities
The Advanced Clinical Practitioner (ACP) will use specialist knowledge and skills to provide healthcare independently to patients. The ACP is accountable for the total episode of care for patients with undifferentiated and undiagnosed needs.
The post holder will act as a role model, delivering advanced practice and promoting clinical effectiveness in care. He / she will be required to work autonomously in delivering independent assessment, clinical examination, diagnosis, clinical decision making and planning of care and treatment, diagnosis, prescribing, health promotion and safe discharge of patients.
The post holder will work closely with all members of the multidisciplinary team and will play an integral role in the operational/strategic development of the service.
The post holder will promote and facilitate evidence based practice and clinical audit and research within the Trust.
The job holder will work as an autonomous practitioner within a defined clinical area, delivering all aspects of management and care to a wide range of patients with varying needs.
Person Specification
Experience (Essential):
* Evidence of significant practical contributions to the development of the specific service within the past 2- years.
* Evidence of continuous professional development and the practical application of new skills to the work place.
* Managerial responsibilities.
* Direct supervision of junior staff, in all professional groups.
* Supervision of students.
* Evidence of lead for or contribution to audit programmes within practice.
* Change Management experience and direct involvement of redesign within a service, including promoting evidence based practice.
Skills & Behaviours (Essential):
* Documented evidence of advanced clinical skills competencies.
* Advanced oral and written communication skills and confidence to discuss patient management with other members of the MDT.
* High level of capability using outcome measures and audit.
* Confidence and ability to accurately assess and manage complex patient presentations to a high professional standard.
* Ability to critique own practice to an advanced level.
* In-depth knowledge in their specialist clinical field.
* Ability to manage time and workload effectively and to work in a high pressure environment.
* Excellent liaison and negotiation skills.
* Commitment to Staff Development and Training.
* Self-motivated and able to work independently and autonomously both within the Trust and with partner organisations.
* Evidence of managing complex patients in stressful and emotional situations where life-changing events are being communicated and clinically managed.
Qualifications (Essential):
* Degree-level registration qualification or equivalent post-registration award Masters Degree in Advanced Practice, or relevant Higher Level degree plus PG Diploma in Advanced Practice.
* Non-medical prescribing.
* Life Support qualifications (or alternative profession specific clinically determined qualifications).
* Registered with relevant professional body.
* Relevant teaching qualification.
