An exciting opportunity has arisen for a Pharmacy Technician to join the Community Agile Pharmacy Service (CAPS). CAPS plays a significant role in supporting patients following an acute hospital discharge or to prevent avoidable admissions during periods of acute illness.
The team supports staff and patients within Active Recovery, Community Nursing, Virtual Wards, Community Therapy and the Community Stroke Service.
The successful candidate will work closely with patients, relatives, and carers to help optimise individualised medication regimens. Our goal is to support patients to reach an appropriate level of independence with their medicines and to support healthcare professionals gain insight into medicines management as part of a holistic assessment process.
