Job summary As a Diabetes Nurse you will provide specialist support, screening services, health education and advice to patients. You will act autonomously within your professional scope of practice providing care for patients both face to face and by telephone consultation. Working alongside the Diabetes Team you will implement improvements to ensure the service is responsive to the Patients needs. Main duties of the job You will need to prioritise and triage the needs of patients accordingly making any necessary referrals for investigation in the appropriate manner.You will be integral to the Multidisciplinary Team acting as patient advocate and providing expert knowledge and guidance. You will also provide supervision and training to peers and junior staff.
