* Provide 1 to 1 emotional support for unpaid carers in Kirklees - we support people caring
for people with mental health needs, people caring for those with drug/alcohol dependency, and those caring for people with dementia
* Provide carers break service by spending time with the person being cared for
* Empower carers and those they care for to manage their wellbeing, to create action plans,
and to engage with their community
* Help with the facilitation of carer support groups
* To signpost people to appropriate other local or national services including mental health
and healthcare services This role will include working with of people aged 18 years and above. You will be working with people with mental health needs, people with drug/alcohol dependency, and those with dementia. You will also be working with unpaid carers.,
* To undertake individual and group risk assessments alongside the service manager with the
people you are supporting.
* To support groups run by Carers Count (including groups for carers and their loved ones)
* Positively represent the service at all times.
* To provide information as requested to develop management reports, monitoring and
evaluation.
* Empower carers and those they care for to manage their wellbeing, to create action plans,
and to engage with their community
* Help with the facilitation and delivery of carer support groups run by Carers Count
(including groups for carers and their loved ones)
* To signpost people to appropriate other local or national services including mental health
and healthcare services
* Provide a person-centred service to individuals receiving support - offering active listening,
empathy, and non-judgemental responses
* Collaborating with other healthcare professionals to support treatment plans for people
with mental health conditions and their carers
* Supporting unpaid carers with issues surrounding mental health and signposting them on to
how they can aid their cared for treatment and support in the community
* Help clients and carers to live their lives as independently as possible and support them in
reaching their potential, through both practical and emotional support.
* Encouraging and supporting the development of personal skills of the carers and cared for
through hobbies and interests and accessing support in the community for these.
* Working alongside relevant support services on cared for care plan (if in place) which may
include encouraging and supporting the cared for to work towards life skills, such as shopping, using public transport and paying for bills.
* Support the carers to work with other healthcare professionals and services to ensure that
all care needs meet the highest possible standards.
* In the case of health-related issues with a cared for, support the carer to notify the clients
services involved.
* Keep all matters always related to clients and/or their families confidential
* Extend support/care period if the need arises
* Promote the service to other services and attend meets and events as and when needed to
do this
* To ensure that accurate notes and records are kept of support sessions.
* To gather feedback and ideas from people accessing the service to help Carers Count
continue to develop.
* Actively participate in training, supervision, appraisals and team meetings to support your
own personal development.

responsibilities aside the case work such as time management, admin and attending meetings All staff have a responsibility toward the effective running of Cloverleaf Advocacy. As such new tasks and duties may develop in the light of experience. These may be allocated where reasonable, after discussion with the postholder.
* Preferably live within or near the Kirklees area. Travel expenses will only be paid within
the contract working area.
* Have recent experience in a paid or voluntary capacity involving a face-to-face role with
people with a mental health need, with dementia, or with drug/alcohol dependency.
* Have experience of working in the voluntary sector, health or social care, preferably with
people with mental health needs.
* Demonstrate excellent communication and negotiation skills, verbally and in writing
* Have good IT skills and be proficient using Microsoft Office 365.
* Be able to develop and continually improve service delivery

OTHER: Temporary contract for 12 months subject to a six-month probationary period
