The role will work within an integrated care system to organise and manage up to 3 Neighbourhoods in a defined locality in Hillingdon (North, Central or South). Using a change management approach and methodology to identify, plan and deliver transformation initiatives to support and enhance Neighbourhood working.
Main duties of the job
1. Proactively engage staff within the Neighbourhood teams and wider stakeholders to enable delivery of initiatives.
2. Committed to working and engaging constructively with internal and external stakeholders on a range of business sensitive issues.
3. Foster and maintain strong links with all services across the PCN and neighbouring networks.
4. Nurture key relationships and maintain networks internally and externally.
5. Close liaison with the Communications and Stakeholder engagement team on public relations and marketing activities.
6. Link with managers and members of other functions to address inter-dependencies and ensure alignment.

Job responsibilities
1. Overall responsibility for the Development and Delivery Plan for the assigned Neighbourhood/PCNs/Practices within your remit.
2. Senior member of The Neighbourhood Management Team who will be responsible and accountable for the design and implementation of the Neighbourhood strategy and development.
3. Working on various HHCP programmes within the Neighbourhoods and individual Neighbourhoods Business and organisational development plans.
4. Working on various Primary Care Network (PCN) programmes that support Neighbourhood working.
5. Working with practices within the PCNs to support their development plan.
6. Using population health data and various BI data to analyse individual neighbourhood development areas and to drive transformation and continuous improvements.
7. Prioritise initiatives in complex and nuanced environments.
8. Develop business cases and development proposals.
9. Facilitate and enable transformational change - build excellent functional relationships with and between multiple stakeholders.
10. Identify process improvement and where possible, streamlined ways of working.
11. Utilise project management and change management methodologies to manage and deliver against the key deliverables (or key performance indicators KPIs) within the project to ensure it is successful.
12. To achieve Neighbourhood objectives through effective facilitative partnership working and engagement to ensure change is embedded.
13. Develop and maintain a collaborative, constructive, delivery focused culture across and between sovereign organisations, without compromising those organisations sense of their own autonomy.

Person Specification
Qualifications
* Degree or equivalent level qualification or significant experience of working at a similar level in specialist area.
* Further professional or academic qualifications in primary care and/or general practice management.
* Evidence of consistent pattern of learning from education, training, and experience.
Experience
* Extensive management experience and/or working supporting senior management.
* Working and delivering change across multiple stakeholders.
* Initiating and delivering projects using Project Management methodology.
* Identifying and analysing various business information data to inform commercial and business need, monitor and track efficiencies.
* Demonstrable experience of delivering measurable quality and cost improvement in complex services.
* Excellent knowledge and proficient user of various IT systems and software programmes, such as Outlook, Word, Excel, PowerPoint.
* Working knowledge of EMIS and SystemOne.
* Excellent written and verbal communication, including listening and imparting information to a wide range of staff and independent professionals at all organisational levels.
* Ability to prioritise, multitask and deliver to timeline.
* Skills for communication on complex matters and difficult situations, requiring persuasion and influence and relationship management.
* Able to operationally redesign clinical/non-clinical processes including producing all supportive documentation, sometimes using data analysis.
* Facilitation, coaching and training skills to enable process adoption and change amongst general staff (clinical and non-clinical).
* Effective interpretation of corporate and contractual objectives into deliverable outcomes.
* Management experience in a provider organisation in healthcare.
