Job summary An exciting opportunity has arisen for an appropriately skilled and experienced registered nurse to join Kirklees Health and Care Partership's Continuing Healthcare (CHC Team) as a lead nurse for the physical disability CHC Team. Although direct experience of working in a CHC team is not essential, candidates must be able to demonstrate knowledge and experience of the CHC legislation and process and have experience of working within the physical disability field of practice. Assessment, including risk assessment, and case management skills are essential.
