Job summary An exciting opportunity has arisen for a part time Stroke Reviewing officer to join our established Integrated Community Stroke Service to deliver six and twelve month post stroke reviews and support the therapy team to delivery rehabilitation into various community settings. Our award winning Integrated Community Stroke Service delivers rehabilitation 8am-5pm, 5 days a week, including Bank Holidays. Weekend working may be required in the future. Main duties of the job The core component of the role will be to provide specialist assessment, signposting and support via a designated reviewing process at 6 and 12 months after stroke This role will also involve working collaboratively within our Integrated Community Stroke Multidisciplinary Team of AHP's, together you all make the difference.
