In your role, you'll be involved in supporting statutory Care Act Assessments for Older People and clients with Physical Disabilities. The objective is to meet 28-day performance targets and reduce waiting times for new referrals. Additionally, you'll handle complex long-term cases, addressing issues such as hoarding, non-engagers, DoLS challenges, and court cases related to welfare disputes. Responsibilities include:
1. Conducting six-week reviews
2. Performing Mental Capacity Assessments
3. Completing Continuing Healthcare assessments
4. Advancing Safeguarding cases exclusively assigned to Social Workers within the team
