We have an exciting opportunity for an Enhanced Clinical Practitioner to join our team, where you can make a positive contribution to patient care and peer support and supervision.
As an Enhanced Practitioner you will be acting within your professional boundaries, providing care for patients from initial history taking, clinical assessment, diagnosis, treatment, and evaluation of their care with the support of the PCN Clinical Lead as required. You will demonstrate safe, clinical decision-making and expert care for patients within the Primary Care Network (PCN), whilst working autonomously and taking full accountability for patient encounters.
The Enhanced Clinical Practitioner will undertake duties within the Practice itself thus ensuring contemporary clinical skills and credibility as well as ongoing integration with the wider team and services. This would include review and care of patients attending the Practice (for pre-planned services or same day urgent care) and include diagnosing and treating patients presenting with minor illness, seeing, and advising people in respect of their continuing medical and nursing needs.
Your work will demonstrate improvement in patient outcomes, contributing to reduce health inequalities across the PCN and improving the efficiency of general practice and delivering a best-in-class service.
Main duties of the job

Job responsibilities
Supporting the overarching Lakeside Healthcare Group strategy and service delivery, the Enhanced Clinical Practitioner provides complete episodes of care for patients with a wide variety and range of presenting problems and health care needs. This encompasses the provision of evidence based, high-quality care for patients whose issues fall within urgent acute episodes, long-term chronic conditions and including health promotion and public health elements.

Mentorship and supervision will be provided by the PCN Clinical Lead. The level and type of supervision will be dependent on the post holders skills and knowledge and determined by the organisations clinical governance arrangements.
Although the expertise of the ECP in Primary Care lies in their ability to operate as a generalist, notably maternity care is the one domain where legislation stipulates that only a registered medical practitioner or a practicing midwife can provide care for a pregnant woman.
As a professional and experienced ECP the post holder will undertake demonstrate:-

Manage undifferentiated undiagnosed condition and identify red flags and underlying serious pathology and take appropriate action.
Develop, with the patient, an ongoing care plan with emphasis on health education and preventative measures.
Actively take a personalised care approach and population centred care approach to enable shared decision making with the presenting person.
Order and follow up on necessary investigations providing treatment and care both individually, as part of a primary care team and through referral to other agencies.
Work collaboratively with other health care professionals and disciplines.
Complete the relevant training to provide multi-professional clinical practice and CPD supervision to other roles within primary care, for example first contact practitioners and the personalised care roles.
