About the Position:
* Lund’s Regional Partnership Program is a state-wide program, employing case managers who partner with DCF Family Services and support caregivers who are impacted by substance use.
* RPP Case Managers are located in local DCF Family Services office and work as an integral part of the intake unit.
* Primary responsibilities include substance use screenings, coordinating referrals to assessment and treatment, and supporting treatment engagement by addressing barriers.
* Services are provided in the DCF office, in the community, and in the caregiver’s home.
* Case Managers also provide consultation and training for DCF staff on topics related to substance use and treatment services.
* Case Managers act as a liaison between the treatment system and DCF Family Services.
* Individual and group supervision provided to support on-going professional development, teaming and to promote/attend to staff wellness.
* This position is Monday-Friday primarily from 8:00am-4:30PM, full time; no weekend or on-call.
* Starting rate of up to $25/hour based on education and experience.
* Lund provides clinical supervision and financial support to attain the Apprentice Addiction Professional certification.
What We Look For:
* Minimum of Associates degree, Bachelor's preferred in Social Work, Counseling or a Human Services related field.
* Experience providing case management to persons in a substance use, recovery or mental health treatment setting preferred.
* Apprentice Addiction Professional (AAP) certificate or the ability to test for certification within the first year of employment. Financial support, clinical supervision, and training provided by Lund to obtain the AAP certificate.
* Experience working with families, multidisciplinary teams, and knowledge of community resources preferred.
* Must be able to demonstrate each of the following:
o Ability to adhere to strong professional boundaries and maintain client confidentiality in line with state and federal regulations.
o Strong problem-solving skills and independent decision making.
o Ability to communicate effectively and respectfully both orally and in writing.
o Skilled in the use of technology including Microsoft Suite programs, Electronic Health Records and video conferencing (Zoom and Teams).
o Strong attention to detail and ability to maintain accurate client records.
o Ability to work independently, as well as on a team, and be self-motivated.
o Ability to handle multiple tasks effectively and accurately, in a busy environment with frequent interruptions.
o Ability to deescalate and resolve sensitive situations respectfully and without judgement; Quick rapport building is essential to this position.
o Regular on-site attendance is expected, and necessary, to meeting the expectations of the job.
o Ability to perform the essential job functions of the position either with or without reasonable accommodation.
