Community Assistant Practitioner under 18s CATAT
CATAT, which was highly recommended for the Working Together for Better Mental Health at the staff excellence awards, is looking for professionals with a keen interest in working with children and young people (CYP) and their families. Our aim is to help them achieve better outcomes for their future while striving to keep them in the community.
The goal of our team is to prevent CYP from having inpatient admissions wherever possible and to support them and their families in a proactive manner. We serve as a gateway in and out of Tier 4 (T4) provision, working alongside core community teams and partner agencies to provide increased support for CYP. This collaborative approach helps with jointly holding risk, collaborative working, and continuity of care.
CATAT has two main pathways:
1. Admission Avoidance: The least restrictive option for CYP who are already open to a core team but need more intensive intervention to support their mental health needs.
2. In-reach/Outreach: Support for young people leaving inpatient settings (T4) to help them adjust back into community life.
Our excellent team has avoided 165 admissions between 07/2022 and 08/2024. If you are a dynamic individual who wants to make a change in services providing support and care to young people and families, consider applying to join us!
Main Duties of the Job
What will CATAT offer?
* Consultation/Advice
* Ensure that children & young people in Norfolk & Suffolk access care in the least restrictive environment possible.
* Case formulation and ongoing case management to establish the CYP care pathway.
* Face-to-face community-based visits
* Attending MDT, LEAPs, CETRs, MALMs, and linking with inpatient and acute wards.
* Therapeutic interventions and approaches related to the expressed mental health needs of the CYP, such as emotional regulation, DBT skills, AMBIT techniques, and risk management.
* A systemic approach to working with CYP, families, carers, and wider professional networks.
* Promote joined-up working and liaise with other involved services and teams across mental and physical health and children's services.

Job Responsibilities
You will be expected to have access to a car and be willing to drive across Norfolk and Suffolk.
The advantage of having a team like CATAT is that we cover all of Norfolk and Suffolk, allowing us to find professionals with experience working with CYP and/or a strong interest in this area.
Don't let the base site deter you from working with us in CATAT. We have investigated this and to support well-being and effective clinical work, several locations for hot desking/room space are available to pre-book when you are on shift:
* Thurlow House (Kings Lynn)
* 80SS/Mary Chapham (Norwich)
* County Hall (Norwich)
Possibly:
After your initial induction period and sufficient shadowing time with the team, if you prefer to use a location closer to your home rather than the main office base, this would be acceptable. However, you would be expected to use this space rather than remote working, and you would also be expected to come into the main CATAT office base 1-2 days a week to feel part of the team, engage in team meetings, and complete your LMS/CS along with other team opportunities.
Person Specification
Qualifications
* Care certificate or relevant equivalent.
* Foundation Degree in Health and Social Care/Mental Health or apprenticeship in H&SC/Mental Health L5.
Experience
* Working with service users with mental health needs.
* Experience working with under 18s.
* Experience working in the community.
Skills
* Ability to communicate widely in a clear manner.
* Ability to self-manage time and meet agreed deadlines.
* Satisfactory verbal and written English language skills.
Other
* Must be able to drive and have access to a car.
* Willing to do long-distance driving.
