LD and SEND Families Social prescribing is a way of engaging patients and their families in primary care with a resource which provides support within the local community.
In addition, it provides GPs with a non-medical referral option that can align to existing treatments to improve health and wellbeing.
The role will provide information and support to patients and their families in addition to becoming the link between the patient, GP and other service providers. The role will require managing and prioritising your own caseload, in accordance with the needs, priorities and any urgent support required by individuals.
Main duties of the job
* Receiving and actioning referrals from GPs, Special Educational Needs Co-ordinators (SENCOs) and wider adult social prescribing teams. This list may vary depending on need.
* Providing personalised support to individuals, their families, and carers to enable them to take control of their well-being and reducing health inequalities.
* Develop trusting relationships by giving people time and focus on what matters to them.
* Taking a holistic approach, based on the persons priorities and the wider determinants of health.
* Co-producing a personalised support plan to improve health and wellbeing, to introduce or reconnect people to community groups and statutory services.
* Having a strong awareness and understanding of when it is appropriate or necessary to refer people back to other health professionals or agencies.
* Ensuring patients and their families are supported and have awareness of safeguarding processes for vulnerable individuals, providing opportunities for the person to develop friendships, a sense of belonging, and build knowledge, skills, and confidence.
* Working together with all local partners by establishing and facilitating the community assets available.

Hinckley and Bosworth Medical Alliance is a GP Federation that supports 12 GP practices across the Hinckley and Bosworth locality, which form 3 Primary Care Networks.
We employ in excess of 45 PCN staff across a number of clinical grades, and continue to grow.

Job responsibilities
Key Responsibilities

Service Delivery
* Building a robust knowledge of health, social and third sector provision available within West Leicestershire and surrounding areas.
* Supporting in the promotion and growth of the service delivered by the role.
* Supporting patients and their families in accessing wider services.
* Establishing sustainable working relationships with the 12 GP practices supported by Hinckley and Bosworth Medical Alliance.
Personalised Care and Support
* Building relationships with patients, their families, and carers.
* Meeting people on a one-to-one basis, undertaking home visits where appropriate.
* Facilitating group meetings with patients, their families and carers, signposting to wider support networks.
* Providing non-judgmental support, respecting diversity and lifestyle choices.
* Helping people identify the wider issues that impact on their health and wellbeing.
* Communicating effectively with patients, families and carers.
* Co-producing a simple personalised support plan based on the persons priorities, interests, values and motivations.
* Where appropriate, physically introducing people to community groups, activities and statutory services.
Support Community Groups to Receive Referrals
* Forging strong links with local VCSE organisations, community, and neighbourhood.
* Developing supportive relationships with local VCSE organisations, community groups and statutory services.
General Tasks
* Producing accurate, contemporaneous, and complete records of patient contact.
* Working sensitively and effectively with people, their families, and carers.
* Encouraging feedback from patients, families, and carers.
* Ensuring that safeguarding protocols are followed for Children and Adults.

Pay
Please be aware that we are awaiting final confirmation from the council in regards to the pay scale for this role. As such, the pay rate may be subject to change.
Person Specification
Experience
* Experience of working in a similar role.
* Experience of working with vulnerable and young people.
* Experience of working in health and social care environment.
* Experience of coordinating services from diverse providers.
* Experience of working in liaison capacity with social care.
* Experience of seeing patients and carers in a practice-based setting, in their own home or in a community setting.
* Experience of working within a mental health, Learning Disability and Special Educational Needs setting.
Qualifications
* GCSE grade A to C in English and Maths.
* NVQ Level 3, Advanced Level or equivalent.
