The Ross Practice in Harlow has a fantastic opportunity for a care coordinator to join our team on a full time basis, for 37.5 hours per week. The role will focus on coordination of the patient's journey to meet their physical and/or mental health needs, working closely with the 3 practices in the primary care network (PCN). In this role, you will : provide extra time, capacity and expertise to our practice team to help us to improve the care provided to our patients. ensure patient health and care planning is timely, efficient, and patient-centred and that all patients receive the best possible care and service. support patients preparing for clinical conversations with a primary care professional or following up clinical conversations they have had. become an important member of your PCN multidisciplinary team (MDT) identifying patients in need of proactive support ensuring the appropriate support is made available to them and their carers as well as any needs being addressed. work alongside a variety of different people working in the practices and across the PCN including doctors, nurses, healthcare assistants, social prescribing link workers, physiotherapists, physician associates, paramedics, health and wellbeing coaches, podiatrists, occupational therapists and clinical pharmacists, as well as the administrative team. About the Candidate If you are a skilled care co-ordinator, with excellent person-centred planning experience and strong local knowledge for accessing key support, then we would love to hear from you. The ideal applicant will have the following skills, experience and qualifications: Good oral communication skills and be able to communicate professionally and effectively with a variety of people at all levels. Good IT/computer skills including Word and Excel. Motivated by helping people with care and empathy and in delivering person centred support in a non-judgemental way. Awareness and understanding of confidentiality Ability to adhere top practice core values and ensure patients, colleagues and other partners are treated with dignity and respect. Demonstrate the ability to show kindness, compassion and a caring manner, with an understanding of human needs physical ,emotional and social Good time management and prioritisation skills, with the ability to work to strict deadlines.
